The AF Form 1297 is a 2 part form that auto populates the information that is given. How many forms actually need to be filled out? One - For the person receiving the equipment to sign and we keep for our records until the equipment is returned? Or, Two - We keep a signed copy and the other copy goes with the equipment? Any help would be greatly appreciated.
Typically the instructions on how to fill out the form and "who" gets "what" are on the form itself (last page). Because of AF Internet and system security, we are unable to access or review the 1297 from the AF ePublishing site without downloading specific software. It identifies the AFIs and regulations which reference and explain the usage of the 1297. After reviewing it, it is our opinion that it probably does not matter who keeps the signed copy or what copy goes with the equipment. But we recommend you contact the AFI 34-204 OPR office, they should be able to answer your question.
