I've been searching the 5000.02 and the DAU Guidebook Ch 1 Program Management, Ch 3 Systems Engineering, and Ch 8 T&E for specific guidance and policy for documenting, tracking, and resolving Deficiencies discovered in T&E. One program I supported separated the Deficiency identification and resolution process from the Programmatic Risk Management process. Deficiencies were classified by severity as Part I**, Part I*, Part I, Part II, and Part III. A current program brings deficiencies found in fligth test into the programmatic risk managmement process and the risk board makes a decision to accept the risk or not. I don't belive the programmatic risk boad has a role in the deficiency reporting process, but I cannot find the source guidance.
I also reviewed the DoD Risk, Issues, and Opportunities guide without success finding Deficiency Reporting information.
Please help refresh my memory by sharing the Deficiency Reporting classification, documentation, and resolution process.
