DD Form 2627 Request for Aircrew Qual and Training

​The Contractor submits this form, along with other documentation required by DCMA INST 8210.1, when requesting approval to enter a new/existing crewmember into training.  Instruction on the use of this form can be found in CMA241, lesson 5, "Contractor's Flight Operations".
