DLA Form 1821, Certification of Qualification

​The Contractor is required to submit this form (or GFR-approved Service equivalent) as part of the Request for Aircrew Qualification and Training (DD Form 2627), and Request for Contractor Crewmember Approval (DD Form 2628) packages.
