Aircraft Operations Survey Template (B-version)

​Use this survey template when conducting your annual or semi-annual Survey of Contractor flight and/or ground operations. IAW DCMA INST 8210.  This template is applicable to contracts under the B version of DCMA INST 8210.1.  Use of this template is not mandatory and is intended as a representation of an acceptable Survey format.  Instruction on Surveys and the use of this template can be found in CMA241, Module 7 "Formal Survey of Contractor Operations".
