DLA Form 1821, Certification of Qualification

​The Contractor is required to submit this form (or GFR-approved Service equivalent) as part of the Request for Aircrew Qualification and Training (DD Forn 2627), and Request for Contractor Crewmember Approval (DD Form 2628) packages.  Instruction on the use of this form can be found in CMA241, Module 5, "Contractor's Flight Operations".
